When you call, email, or use an appointment booking service, Horizon Telepsychiatry may receive the contact and appointment information you choose to provide. Please do not send emergency information by email.

Information is used to respond to your request, coordinate care, and operate the practice. Clinical information is handled through the practice's care and telehealth systems.
