What documentation is available
Psychiatric care often needs paperwork attached to it — for an employer, a university, an insurer or a benefits agency. These are prepared for established patients, based on what the clinical record actually supports.
FMLA and medical leave forms — $150. Federal and state medical leave paperwork requiring a healthcare provider's certification, including the sections on frequency and duration of absence.
Disability and work accommodation letters — $100. Documentation for an employer or benefits process describing functional limitations and what adjustments may help.
School and college letters — $75. For academic accommodations, medical withdrawal, leave of absence or a return-to-study process.
Treatment summary letter — $75. A summary of diagnosis, treatment history and current status, often requested by another provider or a legal process.
Medical records summary — $50. A condensed summary of the record rather than a full copy.
Medication prior authorization — $5. The administrative work when an insurer requires authorisation before covering a prescribed medication.
Fees are charged for the clinician time involved, and are separate from appointment fees on the rates page.
Documentation follows the record, not the request
This is the part worth understanding before you ask.
A letter is a clinical document. What it can say is bounded by what has actually been assessed and recorded — not by what would be most useful to the person requesting it. A provider who writes whatever is asked for produces documents that carry no weight, and that ultimately fail the people relying on them, because employers, universities and insurers can tell the difference.
In practice that means:
Documentation usually requires an established relationship. For most letters there needs to be a real clinical picture to describe — normally a psychiatric evaluation and, depending on what is being documented, a period of treatment.
The content is determined clinically. You can explain what the form asks for and what the deadline is. What the letter says about your symptoms and limitations follows the assessment.
Some requests cannot be met. If the record does not support what is being asked for, you will be told directly and told why, rather than left waiting on something that is not coming.
Timing
Documentation takes time to prepare properly and is not produced during the appointment itself. Requests are best raised at a scheduled visit, with the actual form or a clear description of what the recipient requires.
If there is a deadline, say so when you ask. Forms with fixed submission windows — leave paperwork especially — are easier to accommodate with notice than at short notice.
Emotional support animal documentation
ESA documentation is handled differently from the letters above and is covered separately. In short: it requires a psychiatric evaluation first, and a letter follows only where it is clinically appropriate. It is not a document that can be purchased in advance of an assessment.

Common questions
What you may be wondering.
Can I get an FMLA form completed without being a patient? No. FMLA certification asks a provider to attest to a serious health condition, its expected duration and the frequency of absence — none of which can be answered without an established clinical relationship. Start with an evaluation.
How long does a letter take? It depends on the document and current volume. Raise the request at an appointment and mention any deadline. Leave paperwork with a fixed submission window is much easier to accommodate with notice.
Why is there a fee for paperwork? Documentation is clinician time spent outside an appointment — reviewing the record, completing forms accurately and corresponding with the recipient where needed. Publishing the fees means you know the cost before you ask rather than being surprised afterwards.
What if the letter I need cannot be supported? You will be told directly, and told why. A document that overstates what the record supports is not useful to you — employers, universities and insurers scrutinise these, and one that does not hold up can cause more difficulty than no letter at all.
